Online Form - Swim School Cancellation Form

Cancellation notification forms must be returned at least three business days prior to the next debit date. No refunds or credit will be issued for any unused lessons. Cancellations will not be processed until any outstanding amounts owing are paid in full. 

Withdrawal due to medical reasons will be actioned as soon as a completed form, along with a copy of medical certificate is submitted. 

A payment equivalent to two weeks (per child) will be deducted at the next debit run as cancellation notice for direct debit customers. 

A payment equivalent to two weeks (per child) will be deducted from any refund applicable for upfront customers. Children may attend lessons during the cancellation notice period.

I confirm this is my written notification of cancellation. I understand that by submitting this Notification Form, the above student(s) will no longer be enrolled in swimming lessons and I will need to re-enrol them should we wish to participate in future lessons.*
