By signing below, I confirm that I have read and understood the above Companion Card Holder Terms & Conditions and agree to the following:• I understand the role and responsibilities of being a companion.• I understand that provision of free entry to the MALC is in order for me to provide care and support to the cardholder.• I understand that I can use the facilities and services if I maintain support and I’m the immediate proximity of the cardholder.• I understand that my access may be restricted to a support only role at busy times to maintain safety and fairness, and my access does not necessarily allow for independent participation or workouts.• I agree that if I breach these conditions I may be charged standard entry fees for the facility/service participated in and/or may no longer be able to be a companion to a Companion Card Holder.• I agree to support my card holder in using the Companion Card in accordance with its official Terms and Conditions and MALC Terms and Conditions.
